Your account must be in good standing to cancel. Meaning no declined payments and not during a freeze.

TO AVOID FEES:

Please note if today's date is less than 90 days after the start of your membership, you will be charged an early cancellation fee of $100 to the card on file. You can see that date on your account. (feel free to use this DATE CALCULATOR)

If you are a Siren member, you pay $16 per pass

If you are a Siren Lite member, you pay $18 per pass

After you terminate your membership, you can still buy passes as listed below.

  • $27.00 - 1 pass

  • $75.00 - 3 pass pack ($25.00 /pass)

  • $138.00 - 6 pass pack ($23.00/pass)

  • $252.00 - 12 pass pack ($21.00/pass)

Your membership will end 30 days from the day you submit this form.

You are responsible for any membership charges in the next 30 days

You will receive email confirmation from us in the next 48 hours.

 PLEASE NOTE 

  1. After your last charge you will no longer receive discounts associated with your membership or be able to request guest passes, but you will of course be able to use your last set of class passes associated with your last charge.

  2. You have until the final charge date of your membership to change your mind and reinstate your membership at no additional cost.

  3. If you cancel your membership and choose to reactivate the same membership at a later date, there will be a $60 reactivation fee.

  4. Single passes are currently $27 per pass, and you can purchase them on the "Make a Purchase" tab in Mindbody.
